One option for post-operative pain control is Sprix Nasal Spray or intra-nasal toradol. Toradol is one of the most powerful NSAID's and has none of the negative side effects of opiate based pain medication (percocet/vicodin/norco) such as nausea, dizziness, constipation. Ask your doctor if it is safe for you to try Sprix as a post op pain control medication. NSAID's impact the clotting cascade and increase the risk of bleeding, hematoma, as well as gastritis.

One of the most common short-term complications after augmentation mammoplasty (breast augmentation) is a hematoma. Traction suture is usually placed in the lower lid to evert the lid for a transconjunctival lower blepharoplasty. If there is existing laxity in the lower lid, this traction can accentuate the laxity. If I note preoperative laxity in the lower lid with a distraction test, I will perform a lateral canthopexy or canthoplasty depending on the amount of laxity.

I will offer a midface lift concomitantly with a lower blepharoplasty if the patient has significant malar descent in conjunction with steatoblepharon (fat pads) in the lower lid region. Not all patients with excess skin and bulging fat pads of the lower lids warrant a midface lift. Part of the answer to your question depends on your aesthetic goals and the other on your particular anatomy. I will rarely perform a midface lift in isolation.

I would assume you have spoken to your surgeon about these symptoms. I would try a steroid eye drop to alleviate the itching. I wonder if you may be having an allergic reaction to something. We often use bacitracin ointment in the eyes after surgery and I have had one patient react to this. I treated the patient with steroid drops and the symptoms resolved.

Mild lagophthalmos is normal in the short term after upper blepharoplasty. The key is to keep your eyes protected during this period with artificial tears and ointment at night. I am sure your plastic surgeon has gone over this with you. If you cannot close your eyes 3-6 months after surgery, you may have to have a correction. This problem, if it does not resolve on its own, is fixable.

Mini-facelifts, which in my mind is a skin only procedure, will have a similar scar length to a full facelift. According to your description and timing, I would say this is most likely a seroma. Seromas occur when the skin is dissected away from the underlying soft tissue creating a 'dead space'. The treatment is to monitor for infection, drain, and compress the area. The goal is to have the skin adhere to the soft tissue. It is not uncommon for a seroma to be drained multiple times before this happens.

While most often during blepharoplasty the aim would be to maintain the aperture of the eye, it is possible to alter it somewhat. This would be achieved by a canthopexy or canthoplasty. Altering the insertion of the lateral canthus will alter the aperture or shape of the eye.

It is unlikely the existence of a malar implant would preclude or make it more difficult for you to have a mid-face lift. The soft tissue overlying the implant should be mobile enough to create the lift you want. Either through a lower blepharoplasty incision or through a preauricular (in front of the ear) incision would work.
