26 year old female presented with a primary complaint about the way her nose tilted downward when smiling. This animation effect can be appreciated on profile view. She also did not like the slight dorsal bump as well as the nasal tip definition (too wide). She underwent an open rhinoplasty with dorsal-split hump reduction, septal harvest, cartilage grafting in the form of a columellar strut graft to support the nose, and finally tip suturing for definition (dome-defining, intra-domal). The photos show before and 2 months after the procedure.
