Statement: Dr. Charles Thorne, MD is an internationally renowned, board-certified plastic surgeon who has been in private practice in New York City since 1989. Though he has many categories of expertise, he is best known for his work operating on faces and noses, such as facelifts, rhinoplasty, otoplasty, and other forms of surgical rejuvenation, including pediatric plastic surgery.

Once he graduated with a degree in molecular biophysics and biochemistry from Yale, Dr. Thorne served in the Peace Corps in Ghana before receiving his medical degree from UCLA. The problems you have in the neck, ears and scars have NOTHING to do with which type of procedure (deep plane or SMAS) was done in the cheek! Your issues are related to excess fat removal in the neck and excessive tension on the ears and scars. These errors can be committed in ANY lift. A deep plane lift is not necessarily the solution. The solution is less tension on the earlobes and incisions.

When you say your eyes are dropping do you mean your eyelids? Or the outer corner of the eye? If the outer corner of your eyebrow is too low, then lateral browlift should be a component of any procedure you have. If the corner if the eye itself is too low, then raising it by canthoplasty should be performed. Finally if raising your cheek tissue seems to help, then I would suggest a temporal lift. Midface lifts are generally out of favor.

There are several choices for augmenting the nasal dorsum: septal cartilage, rib cartilage, osseocartilaginous (cartilage and bone from the rib), cartilage from a tissue bank, bone graft from the hip or skull OR artificial material. Hard to say what’s best for you without knowing more of your story. True that the combination of bone and cartilage is subject to less warping than cartilage alone. Breasts come in all shapes and sizes. You appear to have breasts with small diameters giving them this long, thin shape, known as tuberous breasts. I don’t think you will be happy with your breasts with just areola reduction and a mastopexy (lift). In order to have some fullness in the upper half of the breasts and any cleavage, you will need a small implant as well.

The ear donor site is great because there is usually no evidence it was harvested. The cartilage is curved so it is best for use in replacing or augmenting the tip cartilages. Obviously your surgeon would measure your breast diameter, skin elasticity, preference regarding on top of muscle or below muscle etc. But for the sake of this question I would estimate 325 cc moderate profile smooth round silicone gel.

Honestly, you could do the procedures with any time interval in between. If the same surgeon were doing both, you could do at the same time. The labiaplasty is less painful and easier to do under local anesthesia, with or without sedation so it is the more minor procedure of the two. How about having the labiaplasty a week after the breast augmentation so a week later (2 weeks after breast procedure) you feel totally fine?

Your nasal bones are deviated and you have a bit of dorsal hump. That would require revision to improve. Should be relatively straight forward if that is your goal. Your nose is still a little long with an acute angle formed by the lip and nose. If repairing that were added to the correction, it becomes more involved, time consuming and expensive. Yes, you have a reasonably common condition where the bottom half of the ears are totally normal. The upper portion of the ear is large and the rim is not completely formed. The procedure to reshape and reduce the upper portion of the ear is performed under local anesthesia and takes about 30 or so minutes PER ear. Looks like you are a good candidate.

The ears are supposed to be angled slightly. A good rule of thumb is to look at the angle of the bridge of the nose on profile view and compare it to the angle of the ears. Generally the ears are angled slightly LESS than the bridge of the nose relative to a vertical line. Yes, your ears could be made more vertical but it is a rare request. In addition, it wold require an incision in the crease behind the ear which usually heals well but occasionally is hypertrophic (raised and unattractive)…
