It sounds as if you have do have capsular contracture and your implant has a high risk that it may be leaking or ruptured. It is recommended to have the implants removed and the capsule or scar tissue removed as well. You may choose to have them replaced at the same time or wait until a later time to have implants put back in. I would advise against going larger. In your photos your implants appear to be near your collar bone already. Putting in larger implants will mean that the implants will go even higher as well as further out laterally. However,if you do decide to increase your implant volume I think that there is only a small risk of symmastia. You stated that you are happily a 32 DDD so I would embrace that and enjoy what you have.

It is not unusual to have some "lumps" after liposuction but they are usually transient. You may have a segment of fat that has developed a hardening (sometimes referred to as necrosis). It is also possible that a vein could have been injured and as a result you may have a spot of phlebitis or a lymphatic vessel could have developed a lymphocele. Please visit with your doctor and have this evaluated further.

After reviewing your photos, it shows a laxity of your breast skin. You probably have a degree of ptosis (sagging) at this time. You may wish to consider a mastopexy and placement of an implant for upper pole fullness. Adding an implant alone may cause the skin to sag or stretch even more and possibly cause the nipple appear at a lower position that it is now. Discuss with your surgeon the advantages of round vs. anatomic shaped imlants to achieve the upper pole fullness that you desire....

Your left breast does appear slightly higher. I agree with waiting at least 6 months to see if it can correct itself. There are some things that you can do to try and lower the left breast in the mean time. You could try pushing the implant down on the left which may cause the skin to stretch and lessen the indentation line which could possibly be a "double bubble". I agree with the other surgeons assessments. I would also recommend watching physical activities that engage your pectoralis muscle. You may need to alter some of your work outs to prevent further lateral displacement. Wearing a good support bra during the day may also prevent further lateral migration or displacement of the implants.

It would be helpful to see photos of your abdominoplasty incision. "Dogs ears" can occur and may be due to excess skin and/or fat. 16 months is enough time for healing and maturation of the scar. The procedure to revise the excess skin is usually a minor procedure that can be performed with local anesthesia. You should visit your surgeon and discuss what may be done with removing the excess skin and perhaps recontouring if there is any excess adipose (fatty) tissues.
