Plump lips without the duck bill look
Duck Bill usually means too much fillers
Nov 18, 2009
Filling in the lips with fillers can be challenging. There are several parts to the lips, and there is the upper lip and lower lip. The are two main challenges — figuring out how much to inject and in what proportion (ratio). Besides these two challenges, injectors have to figure out what our patients want. Thus, communication with your doctor is very important. It may be helpful to bring in pictures of lips that you like and pictures of lips that you do not like.
Keep in mind that it is…
Obagi Nuderm vs. Clenziderm – Which is better?
OBAGI Nu-Derm and OBAGI Clenziderm have some similar and some different effects
Nov 15, 2009
Both systems will help address acne, but they are different systems. With the Nu-Derm system you get the added benefits of hyperpigmentation reduction and anti-aging effects. Clenziderm will mainly just treat acne. The Clenziderm system does not contain hydroquinone products.

Background info about CLENZIderm from OBAGI

Obagi CLENZIderm is a prescription strength acne treatment that has a patented, solubilized 5% benzyol peroxide which penetrates deep in to the follicle to treat acne…

What is diamond microdermabrasion?
Diamonds are in the tips of the wands used for microdermabrasion
Nov 14, 2009
Where are the diamonds in diamond microdermabrasion?
These diamonds aren’t "spitted out" or in the "dust". The diamonds are embedded (glued) in the tip of the wands used for this form of microdermabrasion.
How is this different?
This form of microdermabrasion is cleaner and less risk of leaving crystals in the skin. It is less messy and the operator doesn’t have to wear a mask.

Hope this answer helps!

Is microdermabrasion safe while on Retin-A?
Retin-A and Microdermabrasion
Nov 13, 2009
The combination of Microdermabrasion and Retin-A can go work well together. We use a crystal-free diamond tipped microdermabrader in our office. Your skin with Retin-A may be more sensitive and to help avoid scabbing and deeper injury we recommend that patients stop Retin-A / Tretinoin for 3-5 days before their microdermabrasion treatment. Patients can then resume Tretinoin the next day after microdermabrasion. Each patient is different and I tailor my recommendations accordingly.

Spider vein treatment side-effects?
Spider vein treatment side effects
Nov 12, 2009
Straight Sclerotherapy (meaning non-foam) side effects include:

Bruising
Infection
Hyperpigmentation
Skin ulceration
DVT
Redness/itching

Straight Sclerotherapy is used to treat spider veins.
Larger veins such as reticular veins may require Foam Sclerotherapy.
In addition to the above risks, there could be the risk of stroke and headaches — the risks are relatively rare.
Please ask your Plastic Surgeon in person regarding your personal risks for the procedure.

Treatment for broken capillaries on face?
Spider veins or broken capillaries on the face
Nov 12, 2009
We would use a 940nm wavelength diode laser to treat small vessles on the face. I believe that IPL is also another great option. We don’t usually inject sclerotherapy for facial veins / capillaries as the blood flow is faster in the face and would “wash out” the sclerosing agent.

Please consult your doctor in person for your specific situation.

What’s the best way to cover up spider veins?
Covering up spider veins
Nov 12, 2009
We recommend treatments of sclerotherapy and/or laser for spider veins, followed by about a weeks worth of wearing compression stocking.
If you just wanted to "cover up" the spider veins, consider wearing compression stockings which are thick enough to "cover" them up.
I don’t recommend the common practice of tanning to help hide spider veins. Tanning in itself has some risks, and tanning then would get in the way of laser treatments. Perhaps for "cover up"…
Is there such a thing as an endoscopic tummy tuck?
Endoscopic Tummy Tuck while a great idea, doesn’t tighten skin nor removes fat
Nov 11, 2009
I work with another surgeon in my practice who has a General Surgery practice and is always gung ho about doing things with a scope (he does a lot of laparoscopic surgeries).
We have discussed the possibility of doing "endoscopic tummy tucks" but we believe that this is best reserved for patients who have muscles that have separated due to pregnancy or age — called rectus diastasis. Sometimes, these patients (or their primary care doctors) think they have a ventral hernia. These…
Juvederm – reuse the syringe for future injections?
Saving Juvederm filler syringes is not our practice
Nov 11, 2009
The manufacturer of Juvederm (Allergan) and other fillers do not recommend saving syringes. It is sometimes tempting to want to save a syringe to see what the outcome looks like after the swelling goes down (about 1-2 weeks); however, the risk of infection is present. We do not save syringes as a policy to reduce the risk of infection or contamination.
We have had some left over Juvederm from some of our injections and have seen that the residual Juvederm dries up pretty quickly. I am not…
Breast augmentation – what are some common risks?
Potential complications of Breast Augmentation surgery
Nov 11, 2009
While breast augmentation is usually a very gratifying procedure for most women who choose to have the procedure, this plastic surgery procedure is not without potential complications. The complications that are associated with breast augmentation can be classified into immediate and long-term, as well as those associated with the surgery itself and those inherent to the implants.
The most common potential immediate complication is hematoma, which is bleeding into the breast pocket or the…
Who can inject Botox?
Who may administer Botox in California?
Nov 10, 2009
I am answering this question with regards to the Medical Board of California:

Who may inject Botox in California?

The Medical Board of California (MBC) states that physicians may inject Botox. Physicians may direct registered nurses or physician assistants to perform the injection under their supervision and with written/approved Standardized Procedures or Delegation of Services Agreements. An unlicensed person, such as a medical assistant, may not inject Botox. The above persons…

Perlane vs. Restylane, what’s the difference?
Restylane vs. Perlane filler injections
Nov 10, 2009
Both

Both are made by Medicis
Both are hyaluronic acid based
Both cost in the $500-600 dollar range in our office per syringe

Restylane

Comes in a "half" syringe (0.4 cc)
Has a slightly smaller needle than Perlane for injections (thus it might hurt less)
Can be injected more superficially
Works better in the lips in my hands
Is a smaller molecule
Lasts a little bit shorter (4 weeks-6 weeks) than Perlane

Perlane

Does not have a "half" syringe
Has a slightly larger…

How much does Perlane cost?
Cost of Perlane Injections vs. Restylane
Nov 10, 2009
The doctors in our office inject the fillers and Botox (rather than nurses or ancillary staff). At this time we charge $525 Restylane and $560 for Perlane. These prices are for each syringe administered (about 1 cc each). Injections with fillers such as Restylane and Perlane require a good amount of time and skill. There is also a good amount of skill employed in giving the numbing injections in addition to topical anesthetic. We don’t charge extra for any of the nerve blocks and/or mucosal…
Can sclerotherapy treat facial veins?
Microphlebectomy works best for facial veins around the eye
Nov 9, 2009
Microphlebectomy surgery involves making a few small incisions (after numbing the area) about 1 mm wide above the varicose vein and withdrawing the vein by hooking it out. The incision is usually so small that sutures are not needed to close it.
There is almost always bruising afterwards in this area.
We don’t like lasers too close to the eye and sclerotherapy may cause blindness as a risk in this area. Furthermore, the increase blood flow in this area may diminish the effectiveness of…
Do Restylane lip injections always give good results?
The answer is “No” for the question does Restylane always give good results – but “usually”
Nov 9, 2009
Lip injections require a good amount of skill and experience from your plastic surgeon or injector. I have found that there are patients with lips that will respond well to fillers, and other patients who end up looking mediocre after lip augmentation. (The answer is "usually", not "always").
The difference is in how much lip existing lip is currently showing, and how well defined is the "cupids bow" area of the lip. There are many different locations and…
