Will Sculptra Give Me Chiseled Look?
Fillers for a Facial Sculpting
Mar 20, 2016
Sculptra is a collagen stimulator that improves volume loss throughout the face.  In general, I like to use Sculptra to soften harsh hollowed facial contours that can occur with aging.  In my opinion, the product does exactly the opposite of providing a chiseled look.  For a more defined chiseled look I typically inject a thicker filler such as Juvederm Voluma or Restylane Lyft to sculpt and enhance the contours of the cheekbones and jawline.

Support garment use after neck surgery
May 10, 2014
When liposuction of the submental neck (under the chin) is performed, it is helpful to wear the support garment postoperatively for a couple of weeks if you can. It will help to minimize swelling in the area. I also like to use these garments for my postoperative facelift patients for similar reasons. If this were liposuction of other body parts (hips, thighs, etc) I would advise wearing the garment for much longer (4 to 6 weeks), but use of a garment on the face is awkward so...
Too young for facelift? 38yrs marionette lines.
Facial volume replacement buys time before facelift needed
Jan 18, 2014
Judging by your age and photos, I would suggest some injectable fillers. Volume loss in the face starts at age 30, and many people lose about a teaspoon of volume per year. Childbirth may have exacerbated this tendency. A lot of lift can be achieved with fillers in the upper cheek area (especially with Voluma or Sculptra), and jawline improvement can be accomplished with additional filler (Juvederm Ultra Plus or Perlane or Sculptra). If you replace the lost volume...
Upper eyelid surgery at 26? Should I do it and if so, do I wait until the botox wears off in my for
Botox and Blepharoplasty
Dec 25, 2013
Upper eyelid Blepharoplasty should NOT be done until all of the Botox effect has worn off. Botox causes the brows and lids to take on a different configuration, and this may distort the planning and ultimate result of surgery. I always advise waiting AT LEAST four months after Botox treatment before undergoing eyelid surgery. On the other hand, Botox can enhance the ultimate look of the upper face and brows if it is used after the eyelid surgery has healed. Also, I...

How fat grafts and stem cells can improve facelifts
Nov 8, 2013
Stem cell technologies are still in the early phases of development. Having said that, we do know for sure that body fat does contain stem cells, which help provide growth factors and long-lasting rejuvenation for patients undergoing facial fat grafting. I often add a patient's own fat grafts when I perform facelifts. This improves volume loss ( a big part of what makes a face look old), and adds the patient's very own stem cells (which produce the patient's very own growth factors). ...
Is it unwise to consider fat transfer over sculptra to restore volume to my cheeks and temples in o
Fat Grafts vs. Sculptra: pros and cons
Oct 23, 2013
Sculptra is a great way to soften harsh contours in a very lean face. Fat grafts also work well, but actually are not always a less expensive option. When I perform fat grafts, I always use a sterile operating room, and my patients are under twilight anesthesia. The fat graft procedure, when done properly, often results in significant bruising and swelling. Results from new techniques in fat grafting are more reliable than they used to be, and can last for many years. Most of my fat...
Should I Get a Cheek Lift Instead of Continuing to Fill Nasolabial Folds and Cheekbone Area with Ra
Nasolabial fold correction
Aug 21, 2013
Volume loss is the reason for nasolabial fold formation. In a young person ( in their late 30s or early 40s), the best way to improve the nasolabial folds is to replace volume. Thicker Hyaluronic acid fillers such as Perlane or Juvederm Ultra Plus are a great way to get a lot of lifting and filling. When I inject to improve the nasolabial fold, I will often fill a little higher on the cheek bone area as well to get that extra bit of lift that really helps a lot....
Does a Facelft Permanently Damage Tissue?
Facelift Surgery Complications Extremely Rare
Jun 9, 2013
Facelift surgery is designed to rejuvinate, tighten, and enhance the contour of facial skin, muscle, and fat. Any surgical procedure, including facelift, can result in complications such as scarring, infection, bleeding or nerve injury. However, these are extremely rare, and most problems are treatable. Facelifts do not cause damage to the skin, nor do they cause thinning of the skin. Most medical estheticians understand this, and often will work along with plastic...
What Eye Cream Could Be Useful?
Tips for lower lid dark circles
May 24, 2013
Dark circles under eyes can be improved by reducing swelling of the muscle tissue under the delicate eyelid skin. To do this, it may be helpful to avoid very salty foods, and to keep allergies under control as much as possible. Application of cool compresses for 5 minutes in the morning before using makeup may help a bit. In my office, we have had excellent results for overall lid improvement with Obagi Elastiderm eye cream. Our patients and staff have tried a lot of the...
Smoking & Facelifts?
Facelifts and Smoking
Mar 13, 2013
Smoking causes complications in facelift surgery specifically because facelifts require elevation and movement of large flaps of skin and muscle tissue. These flaps rely on good blood supply for survival, and smoking interferes with that crucial blood supply, leading to problems such a tissue death (necrosis and gangrene), infection, and increased scarring. The same problems apply to other plastic surgical procedures involving large flaps of skin such as abdominoplasty and breast...
Risk for Sculptra with Keloid Conditions?
Fat grafting is a possible alternative to Sculptra for keloid prone individuals
Feb 17, 2013
Sculptra is a collagen-stimulator, and as such may not be suitable for individuals prone to keloids. A possible alternative to Sculptra for volume enhancement is autologous fat grafting ( the fat suctioned from an individual's own abdomen or other donor site using a very small cannula, and then injected in the areas needing augmentation). However, if you are extremely prone to keloids, even a small wound or incision may precipitate a problem, and may require monitoring and...
My Eyes Have Become Uneven- Not Sure if It is my Eyes, Eyelids, Brows, Etc?
Eye and brow assymmetry are common concerns
Jan 24, 2013
Everyone has a certain amount of assymmetry in their facial features, and we tend to notice it especially in our eyes and brows (paired features). Judging from your photos, the degree of assymmetry that you have does not warrant treatment in the near future. As you age, there will be further changes in the eyelid and brow postitions, and then it will be appropriate to consider injectables such as Botox, or even surgical options. In the meanwhile, you can treat swelling by...
Can my face be lifted with a laser?
Facelift works best for rejuvination of jowls
Dec 16, 2012
Laser treatments are generally best for improvement of surface irregularities of the skin. Some types of lasers (CO2, for example) may provide a very small amount of tightening as well, through heating of the collagen layer. However, in order to correct visible jowls and sagging of the cheeks and neck, the best treatment option is a facelift. Facelift surgery (which should include SMAS and platysmaplasty) will not only tighten neck and facial skin, it will also correct drooping...

How to decide what size implant is best for you
Nov 23, 2012
A good way to get a rough idea of how a breast implant would fit for you is to try on sample implant sizers at your plastic surgeon's office. In my practice, we provide patients with this opportunity at the time of the preoperative visit. Trial sizers are placed in the bra that we purchase for each patient, and our patients can decide whether they like the look of the various implants in the bra, with and without a shirt if desired. Naturally, I add my advice and recommendations...
Will or Can Another Doctor Provide After Care in the US for Sugery, You May Have Had Done Overseas?
Travel abroad for cosmetic surgery is risky
Nov 14, 2012
Traveling abroad to undergo a surgical procedure is risky, and if you have a problem after you return home it may be difficult and expensive to have it taken care of. Many plastic surgeons, myself included, generally recommend that you get followup care from the surgeon who performed the initial operation. Your operating surgeon is the best person to manage complications, as he or she knows exactly what was done at the time of surgery. If you travel abroad for surgery, it...
