The defects can worsen overtime especially when there is a history of weight gain and the skin laxity which increases with weight gain as well. some of these defects can be made better. Im not sure how much scarring or fibrosis you have as it will need to be evaluated in person. Breaking up that much scar tissue is not gonna be easy esp if a heat or ultrasound based device is used. it only creates more scarring. I would have to see and examine in person to see how much fibrosis there is and how much fat loss there is. and then determine the plan.

From the pics it looks like there is some thicker scar formation (hypertrophic scarring) which is very common in ear surgery. it will require a surgical procedure (another otoplasty) to remove the scarring, and to smooth the helical rim. Sorry this happened but its not uncommon to have this problem after ear surgery. To some extent the fat grafting will help. id say perhaps by 50%. the skin is a bit on the loose side and for that you will need some laser treatments to help with skin laxity and that together with fat grafting will help to restore most of the problems you’re having. I am not sure exactly how this defect came into existence but let me ask, did you have an incision inside the belly button as well? The scar thats there now is quite deep and tethered to the underlying tissues. You also have some stretch marks which make it harder for skin to retract. It can be improved upon, for sure. Ideally this should be done in more than 1 attempts to correct the underlying deformity. the ear lobe can be reduced that’s a pretty common procedure. the helical rim will entail some reshaping. it can be done but the total cost is hard to determine as it will depend upon how many small procedures you will need to make sure it has the shape you like.

The pain you’re experiencing might be due to scarring. as the ear pinning surgery can sometimes heal with thicker scars. they may not be visible on the surface but sometimes you can feel them. You might be able to ease the pain by simply revising the scars. Im not sure if that will be able to solve your problems but its definitely something u can look into.
