It appears from your photo that your breasts look normal and should function normally if you decide to breast feed. A small to moderate sub-muscular breast augmentation with give you more fullness but may not bring your breasts closer together. Larger implants tend to look less natural.

Although I do not have any measurements of your breast width, your photo indicates a somewhat narrow chest wall. If you want to look natural, I would suggest getting an implant that fits your dimensions rather than focusing on the end volume. Please keep in mind that even though you would like the lower portion of your breasts fuller, a 500cc implant is also going to make the upper portion of your breast fuller. Discuss all your options with your plastic surgeon before surgery.

Although your frame can accomodate the 450cc implants, I am concerned that your young tight skin may not. If you want to look natural, I would suggest less volume and would advise that you not be too concerned with actural number, rather the expected outcome and appearance.

At 23 you have healthy tight skin and muscle which could take 3-6 months to stretch and relax to accommodate a 330cc implant. They should settle over time and look more natural.

Your photos do indicate a moderate degree of ptosis and you would benefit from a breast lift in addition to augmentation. If, however, you decide against a lift and understand that an augmentation alone is not ideal, you may want to consider a shaped silicone implant under the muscle to help give you some upper pole fullness.

I would not recommend overfilling your existing saline implants as that much fluid may compromise the integrity of the implant. To give you more upper pole fullness without a lift, you may want to discuss the new shaped silicone implants by Sientra or Allergan with your plastic surgeon.

I think your expectations are realistic and I believe you should be able to achieve fairly symmetric breasts. You most likely will require a reduction in the size of your right areola which would also lift your breast. Once your breast width is measured by your plastic surgeon, you should be able to determine an appropriate size implant.

Some parts of the body have more fibrous tissue surrounding the fat that may make liposuction more difficult. More bruising sometimes occurs. Good results are still possible and you should discuss the procedure and the outcome with your plastic surgeon.

If you are still experiencing drainage from the incision around your nipple after 3 weeks there may be reason to be concerned. You should be evaluated by your plastic surgeon for infection (cultures should be taken of the fluid) or possible hematoma or seroma. Your plastic surgeon should be able to discern the cause after a thorough exam.

It appears from the photos that your skin is tight. In my opinion, you should wait several more months to allow your skin and muscle to stretch before considering larger implants. The implant manufacturers sometimes give discounts to exchange implants for a larger size. Have your plastic surgeon contact manufacturer for financial consideration.

It appears from your photos (especially lying down) that you have some capsular contracture. I do not see evidence of tubular deformities. I would suggest replacement of saline implants with silicone gel (slightly larger) under the muscle and reduce the size of your areola. You should have a more natural result.

Although your age is not specified in your question, any breast pain that does not subside in 1-2 weeks should be investigated. If you are right handed, then certainly a muscle pull is a possibility. You should be evaluated either by your PCP or your plastic surgeon if the pain persists.

There appears to be a significant improvement in your breast shape after the revision. The breasts appear much more natural and although they are closer together, I do not think you will develop symmastia. I would not recommend any further surgery or implant replacement at this time as the improvement would be limited at best.

A 120cc implant is fairly small. If you were a small B pre-op, then adding 120cc should increase your cup size to a medium B or B+. There should be minimal discomfort after the implant exchange surgery. Making your areola smaller should improve your overall balance as your volume will be less. The scars should heal as well as your post op photo illustrates.
