I'm sorry you aren't happy with the results of your reconstruction. Most of the time, the final implants will appear smaller than the tissue expanders because the expanders are so hard, they hold the skin up more than the softer implants. You shouldn't be worried to tell your surgeon what is bothering you- we don't take it personally and I'm sure he will want to help you feel the best about your reconstruction that you can. You are struggling with a hard decision. Unfortunately, each type of breast reconstruction, whether it is an implant, latissimus flap, or DIEP flap has some sort of tradeoff associated with it. In the case of the flap surgeries, one of the tradeoffs is a scar located elsewhere on the body. It can be difficult to decide an implant size. Most plastic surgeons will offer sizers in the office. We also have a 3D camera that can give a good idea of how different implant sizes will look on your body. I also tell people that depending on your body and breast tissue, you may not be a good candidate for the size implant that you wish. Placing implants that are too large for your frame and amount of tissue covering the implant can lead to problems later on.

There are pros and cons to both locations of the implant. Typically, placing the implant under the muscle can help disguise the implant at the top, especially if you don't have a lot of breast tissue, however, you will notice that the implant can move when you use your muscle. A lot depends on your body type and the size implant you are hoping to have. Your doctor should be able to discuss both options and help you decide what might be best for you.

As long as you are healthy and fit for surgery, there is no reason you can't have a breast reconstruction 10 years after your mastectomy. There are many options available to you. If you have to travel for your surgery it can make it more challenging, but I have many patients who come from far away in order to have breast reconstruction!

The most common procedures I perform for a DIEP revision are touchups of the abdominal scar, removing excess tissue from the sides of the chest, minor adjustments to the position or size of the flaps for symmetry, or fat grafting to the upper part of the reconstructed breast. You should think about what, if anything, bothers you about your reconstruction and talk with your doctor about what could be done to address it.
