At 200 lbs your BMI is just under 30. Different surgeons have different opinions, but to me this would be a reasonable place to consider surgery especially considering that you are down from 275. Would lower BMI be better? Sure… However, a weight of 160 may not be realistic without a measure such as gastric bypass.
As to your question about drains, I offer no drain tummy tuck commonly but generally place drains if someone’s BMI is at or above 27. This is due to the fact that the seroma…
Pseudo seroma 11 months post op. Is there a risk in not removing it?

The area has probably developed into a chronic seroma which is not likely to ever resolve at this point. Probably no risk to leaving it except the persistent lump. Unfortunately, the only way to resolve it now is to surgically excise it, which involves a surgical exposure similar to the tummy tuck dissection that you originally had. Removal may also result in a contour deformity–a depression where the lump is–which you might like even less than the lump. Hope that helps and good luck!

With that much weight loss, you might be happiest with a fleur-de-lis type abdominoplasty. I recommend being examined by a ABMS board-certified plastic surgeon who will be able to assess how much extra skin you have and advise you on pros/cons for you! Based on the photos, there is a good possibility.

I expect you would be happiest with a full (standard) tummy tuck as opposed to a mini. Even if you do not have separation of the rectus muscles (diastasis), tightening these (which is typically not done with a “mini”) is going to help get to the nicer contour that you are seeking.

Most likely you have an umbilical (belly button) hernia. Sometimes these can be repaired at the time of tummy tuck (if small) and some will require repair prior to the cosmetic procedure. Good luck!

Unfortunately there’s no easy answer here…the telangiectasias that you have are common after steroid injection for hypertrophic scars or keloids, as is the soft tissue atrophy that you are seeing as a “dent”.
Scar revision can be helpful for situations like this but there’s the possibility that the hypertrophic scarring can recur. Another option would be scar revision in conjunction with a short course of radiation treatments. Good luck!

Unfortunately if you are someone who is known to form keloids, there is a possibility of keloid formation at the umbilical scar too. These can be so thick and unsightly that it can detract from an otherwise nice result. They can always be treated with steroid injection and scar revision if needed. Good luck!

The amount of skin removed at the side is always less than in the center, so unless the scar continues further to the side (or even all the way around as in circumferential lifts) it’s probably going to be looser there.
That being said, you look like you do not have a great deal of laxity toward the side so it’s probably going to look very nice. I recommend asking your ABMS board-certified surgeon specifically this question and how far the scar will go. Hope that helps and good luck!

Standard vs “extended” could be the same procedure depending on who is answering the question…this is why an in-person consultation with a ABMS board-certified plastic surgeon is so important! That way you will know exactly what is being desribed.
With your photo, it is clear that you will need skin excision along with tightening of the abdominal wall and a standard scar pattern of lower horizontal scar and scar around the belly button. Some docs may call it “extended” if the scar goes…

Thanks for your question! Although there is some soft tissue laxity and excess that will improve with blepharoplasty and facelift, in my opinion the volume loss is what is contributing significantly to an aged appearance. If the hollowing in the cheekbone area, lips, and other areas is not addressed, you would be missing an opportunity to restore youthful volume. I assume you are referring to platysmaplasty–procedure performed to bring the left and right parts of a platysma are brought together. This can be done with either permanent or absorbable suture; after 2-3 months and probably sooner, the muscles have scarred together enough that the suture is no longer really needed. This is why many surgeons use absorbable suture for this repair. Hope that helps!

Filler targeted to the brow roll (between the brow and upper eyelid) may help fill in the hollow appearance that you are seeing. The photos display very small, so it is somewhat difficult to assess well. However, you appear to have a downward cant at your lateral aspect of the eye, which is likely congenital and related to the underlying skeletal structure. That can’t be changed without very invasive procedures. However, blepharoplasty is likely to help with opening the eyes fully and potentially ptosis repair, if indicated. Nearly everyone has some asymmetry which needs to be addressed during these procedures and the exact procedure needs to be tailored to you after an exam by a board certified plastic surgeon. That being said, in this photo it looks like your left upper eyelid and eyebrow sits a little lower than the right. It might be OK to only address the extra skin and not address the brow, but if you are already always holding the brow up with the forehead sometimes this leads to additional brow ptosis…

Based on your history and photographs I believe it will be unlikely you will be happy long-term with a periareolar approach to a mastopexy/breast lift. One can only be sure on exam, but the amount of skin you are describing and suggested in photographs cannot be dealt with utilizing a periareolar-only approach. It may look good for a month or 2 but there will be substantial distortion and flattening of the apex of the breast as a result of the excessive tension…
