Marionette lines and jowling almost always result from sun damage to the surface, stretching of the skin, an actual loss of fat, or both, so suctioning fat out usually does not help. This kind of deflation and sagging is, of course, a common problem. A careful exam by a Plastic Surgeon should lead to a discussion about your options, including a breast lift and/or implants. I agree with others that 3D imaging is very helpful in helping you understand the choices, and would always use this with my patients for this problem.

Differences in swelling and pain in the first few weeks after surgery are completely normal. These will normally slowly go away over the first four to six weeks, but sometimes take longer. You shopuld notice that you feel and look better every week!

While I would always want to evaluate a patient in person before making a plan, your photos are consistent with stretching of your skin as the implant has moved downward on your chest. As the implant has settled, some of the stretched skin is now showing above as it has been "left behind." Although photographs are never a substitute for an in-person exam and discussion, most plastic surgeons would probably tell you that you will get the best final shape with a lift. Having said that, I am always comfortable with a patient trying out the compromise of a dual plane or submammary (in selected cases) breast augmentation without the lift. There are several factors which make the lower lids appear to have "dark circles". Actual dark pigment in the eyelid skin itself can result from chronic allergies, inflammation, heredity, and sun exposure. Firmness early is completely normal, and not of concern for contracture 9 days postop. With one side much more painful than the other, I would want to see one of my patients more promptly however to check for rate problems. At a minimum give your surgeon a call to discuss it with them.
