Answer Snippet
My breasts are uneven after BA and it's making me depressed. What can I do?
Asymmetric breasts
Aug 6, 2015
Your breast buds grew in two different directions, specifically in 3D. This is very common. You had two completely different breasts in terms of position, shape, areolar size and direction, and inner anatomy. Ignoring the nipple areolar size and location, you have a good result. The augmentation makes the two totally different breasts look much more symmetric. You are lucky, the augment could have amplified the asymmetry. Ask your doctor if he could reposition the...
I am going with 350cc silicone implants. Why do doctors prefer under the breast incision?
Inframammary incision
Aug 3, 2015
I do surgery on many women of color, including 'Mediterraneans'. The incision tends to be hyperpigmented so I do more periareolar incisions on these women. The scar blends nicely into the bumpy darker areola.
Follow up. Have I got the wrong implant?
Ripples
May 18, 2015
I encountered this in a thin European female. She underwent submuscular saline implants. The correction was simple. The patient agree to replace the saline implants with silicone and agreed to have a acellular dermal matrix sewn in to the top hemispheres of her breasts. The combination solved the problem. Your implants are silicone but they underneath the skin of an athlete, who's skin thickness will change with the extent of the athletic activity especially if there is...
Can pressing the hump on nose with fingers regularly flattened the nose bone?
Pressure flattening of the nose
May 18, 2015
I do a lot of noses in my community. Pressure does not work. It is not uncommon to mismatch the take down of the bony part versus the supratip area. This is the most difficult part of the surgery. You can verify this from your profile. Your surgeon thought he/she had a satisfactory matchup only to find the mismatch postop because of the way your body healed. Fortunately, the correction is relative easy and lost less expensive and debilitating. You can tell...
Are numbing cream as effective as local anesthetic?
numbing cream
Apr 27, 2015
I have a numbing cream that I have formulated with tetracaine and I have not performed a block when I inject fillers or do laser. There are great topicals out there.
Seroma or hematoma - which is worse? Which recurs more and why?
seroma nd hematoma
Apr 27, 2015
I just had a hematoma from a patient who fell 24 hours after surgery. Sometime just a generalized oozing (of small blood vessels) will cause a hematoma to form. The hematoma was evacuated and a drain was placed. I have not had a surgical seroma in the past 10+ years and they should be drained also. With 10-20 cc of sero-sanguinous fluid, the drain could probably could come out and you will not have a recurrence.
What would be needed to fix these?
Improving drooping breasts after augmentation
Apr 24, 2015
You could benefit from a breast lift with minimal adjustment of the breast position. In other words, a lift would bring the downward nipples and the "empty" lower pole of breast tissue up to "dome" level of the implant. The second surgery should could be considered a "second stage" of the corrective process.
