The primary issue related to loose lower eyelid skin is loss of skin elasticity. Phenol peels and lasers are both excellent ways of restoring collagen and elastin in the skin to tighten and smooth the skin but they both carry some small risk of worsening your dry eye symptoms.

You are right, a breast augmentation will increase the diameter of your areolae and since you feel your areolae are already large, you may very well want to consider having them reduced at the same time as your implant surgery. When I combine these procedures I use a separate incision in the fold underneath the breast for placement of the implants in order to minimize the risk of nipple numbness, infection and capsular contracture.

Breast reduction after lumpectomy and radiation does carry a higher complication rate in general but it can be safely done if certain principles are followed. The surgery should be delayed until all of the acute inflammation and swelling from the radiation have subsided and the surgical technique should employ shorter, wider pedicles and minimal undermining of the breast flaps.

There is an objective way to determine if you really need a lift. If the nipple to infra-mammary fold distance measures greater than 10 centimeters with the skin on stretch or if your skin stretch at the nipple when pulled forward is greater than 4 centimeters, you will need a lift in addition to breast implants. I recommend a consultation with a surgeon certified by The American Board of Plastic Surgery.
