Fish Oil Injections for Breast Growth?
Injecting Strange Materials for Breast Enlargement
Nov 19, 2012
Hello,

Funny how urban myths can assume credibility. As my colleagues below and above have / will state, the only known safe materials to make a human breast bigger are gel or saline implants, or your own transplanted fat.

Drains After a TT?
Drains Are Worth The Bother After Tummy Tuck And Liposuction
Nov 9, 2012
Hello,

Don’t get too stuck on details like drains. Considering a moderately big operation like Abdominoplasty and liposuction, the burden of drains is a small part of the total recovery after surgery. Think more about wound care, nutrition, pain control, clot prevention, and activity. Like planning for a baby, planning for cosmetic surgery should be reasonably predictable. One or two drains is commonly done, lasting about one or two weeks.

Good luck.

Dissolve Restylane Before Bleph?
Timing Lower Blepharoplasty After Dermal Filler Therapy To The Tear Trough
Nov 9, 2012
Hello,

Unless the volume of remaining filler is thick and palpable, I don’t think recent filler therapy will influence the performance of lower blepharoplasty by your plastic surgeon. Most of the tissue manipulation is mid-lid, between the lash line and the bony lower orbital rim. The tear trough is at the lowest part of the blepharoplasty field.

Good luck!

Is a Tummy Tuck and a Breast Implant with Lift to Risky to Do in One Procedure?
Combining breast augmentation and tummy tuck
Nov 4, 2012
Hello,

Combining two moderate operations is a matter of scale, proportion, and complexity. We always weigh efficiency of the time savings of combining operations versus the increased risk and stress and possibly tougher recovery.

If it patient’s ambitions for augmentation are modest and she is very suitable for an abdominoplasty, i.e., good muscle tone and largely lax skin without a great deal of fat, then the operations being combined is generally a pretty good fit…

Can I Get Breast Implants at 16-18?
Teenage Breast Augmentation
Nov 4, 2012
Hello,

I think that augmentation at too young an age can be unsatisfying over the long term. First, the chest wall and breast tissues can still be growing and maturing. The expectation of a large-ish implant under immature tissues can actually cause atrophy and thinning of the overlying tissues, leading to faster fatigue of the tissues and loss of elasticity. Second, psychologically the young patient hasn’t lived in her skin for very long in an adult state, so expectations about surgical…

How Will DDD or E Cup Look Like After Breast Reduction to C Cup?
How Does Breast Reduction Influence Breast Size?
Jul 28, 2012
Hello,

Most colleague plastic surgeons will waffle on this seemingly simple question, myself included. I specifically state that the purpose of breast reduction is reduction in symptoms, such as chronic back neck and shoulder pain & rashes underneath the breasts, not a specific bra size.

The amount of breast tissue and excess skin taken away and their influence on bra size vary according to the tissue itself as well as the overall shape and dimension of the chest wall underneath…

Breast Reduction and Lift
How does breast shape change from breast reduction?
Jul 28, 2012
Hello,

The usual result from breast reduction is an upward lifting, compaction of looseness, and reduction in overall volume. There are quite a few ways to do this but most involve removing excess skin and internal breast tissue, and repositioning the nipple to be directed forward rather than downward. The best method for you is difficult to specify sight unseen. Good luck in your journey to better comfort and mobility.

When Does Swelling After Breast Augmentation Begin to Subside?
When Does Swelling After Breast Augmentation Begin to Subside?
Oct 30, 2010
Hello,

After breast augmentation, the breasts will become moderately swollen and quite a bit firmer than normal, both from the presence of the addition volume of the implant, and also from the general puffiness that accompanies handling / traumatizing the chest wall tissues. The general direction the breast goes is less firmness, with relatively small decreases in absolute size as healing progresses.

Anesthesia for Breast Augmentation
Anesthesia for Breast Augmentation
Oct 30, 2010
Hello,

For primary breast augmentation, I ask the anesthesiologist to provide general anesthesia with airway control, usually a LMA tube. This permits me to focus on optimizing the operation, content that the patient is safely ventilated.

Sometimes in revisional implant surgery with an already-established pocket, and consequently much less dissection needed, local anesthesia with / without IV or oral sedation works just fine.

The most important factors are safety and…

Is It Possible to Reduce Ear Lobe Size?
Is It Possible To Reduce Ear Lobe Size?
Oct 30, 2010
Hello,

One condition to treat is the torn earlobe or the multiply pierced earlobe. Depending upon the degree of tear or multiplicity of pierce holes, the first goal is to convert the earlobes to a simpler, less pierced or unpierced state. Thie involves surgical removal of the skin which lines the pierce tract(s), and sew the fresh edges of the former pierce together.

The other condition is reduction of the enlongated earlobe, usually encountered in older patients. In…

How Many Units to Botox Crow’s Feet?
How much Botox to treat Crow’s Feet?
Oct 30, 2010
Hello,

The first time I treat a patient for crow's feet in the area of the lateral lower eyelid, I usually start with 5-6 units per side, spread over two or three injection sites. The dosage can be raised for more dramatic and sustained effect, but it is important not to weaken the muscle that crinkles the eye so much that the lower lid sags, which can be uncomfortable or unattractive.

How Often Can We Use Botox?
How often do you need Botox treatments?
Oct 30, 2010
Hello,

I generally see patients having a return of overactive muscles of the brow, forehead, or crow’s feet, at around 4- 6 months after treatment. Over time and multiple repeat treatments, there is often a longer period of effectiveness, but the same dosing of botox is needed, just further apart in time.

Is Breast Augmentation Through Tummy Tuck Incision Possible?
Augmentation via abdominoplasty route
Sep 23, 2010
I would be conservative and play to the strengths of each operation. Trying to make a precise implant pocket from the upper abdomen dissection field seems like an invitation to inaccuracy. While buying an external scar for the augmentation is a drawback, the profit is a quicker operation with greater accuracy and symmetry.
What Types of Breast Implants Are Available in the US?
Breast Implants Which Are Available For Augmentation In The USA
Sep 23, 2010
Thank you for your question. Both silicone gel and saline implants are available for breast augmentation in the US, provided you are 22 years of age or older.

Be mindful that the FDA has charged plastic surgeons with monitoring implants for their longevity. Silicone gel implants have very thick cohesive gel, and a tear in the implant skin will frequently not be apparent from the outside. Therefore patients who select silicone gel implants are well served by having periodic MRI…

Who is qualified to administer cosmetic fillers?
Who should inject dermal fillers?
May 8, 2009
The leadership in the specialty of Plastic Surgery believes that injection of semi-permanent fillers to change the contour of the face or reduce folds or wrinkles should be done by a physician.
The impact of misapplied filler, compared to over- or under-treatment with Botox, for example, is more significant. Too superficially injected leads to bumps and nodules; too deeply injected lessens the power of the filler to correct lines and wrinkles.
With training, anyone can successfully and…
