Should I absolutely not have upper blepharoplasty if I already have dry eyes?
Possibilities Exist
Jan 22, 2018
It all depends on the severity of your dry-eye condition. For instance, many patients will complain of "dry-eye," yet wear contact lenses. There is a great degree of variability when it comes to dry eyes. Mild to moderate dry-eye patients can often times have conservative upper-lid surgery, successfully. In these circumstances, one can expect to need more aggressive treatment of their dry-eye treatment for a period of weeks to months after the surgery. If an appropriate surgical procedure...
I am very unhappy with my breasts at 47 years of age and wish to know what to do about them? (
Perfect Candidate for Breast-Lift w/small Augmentation
Jan 21, 2018
You are the perfect candidate for a breast-lift with a small implant. The addition of a small implant is primarily for shaping purposes, to give you upper pole fullness and cleavage. Small implants have a minimal, and very subtle effect on the size of the breast. Most breast-lift procedures can be done with an incision around the nipple area, and a vertical incision from the nipple to the crease below the breast. 95% of these scars fade out over the period of a year or two. Less than...
What type of facelift properly addresses the marionette lines and jowls?
Choose the Surgeon, Not the Technique
Jan 21, 2018
All modern facelift techniques address the marionette lines and jowls. Human anatomy is consistent. All our pieces and parts are in the same place. The difference in these multiple, different facelift techniques is how you approach them. The primary distinction is whether you approach them superficially, or deeply. The ultimate outcome from these varying procedures is the same providing they are well done, and address those foundational structures in the face and the neck. Public...
How can this rhinoplasty be reversed? What tip of grays are needed
You Can Go Back
Jan 20, 2018
Revision rhinoplasty often revolves around replacing what was removed in the primary operation. While you may not be able to restore this look, completely, the use of grafting can reverse most of what's occurred in the tip of your nose. The grafts used for this sort of procedure would most likely come from either your nasal septum or your ear. Always consult with a board-certified surgeon experienced in revision rhinoplasty for this sort of issue.

I need a 3rd revision. Should I have a 4th rhinoplasty for warped rib cartilage graft & droopy
Focused Correction
Jan 20, 2018
With each nasal surgery, the procedure becomes more difficult, less predictable, and more prone to complications. Autologous rib grafts have the unfortunate tendency to warp. When faced with a situation like this, focusing on the good parts of your nose will help you better define the parts that really need to be considered for the next procedure. I would consider preserving the autologous rib graft and disguising the warped appearance with injectables or further grafting. The addition of a...
Is it normal for my nose to look big after rhinoplasty 2 weeks after surgery?
Patience
Jan 20, 2018
As this question appeared in the revision rhinoplasty section, I'm assuming this is a secondary nasal procedure. The fact that the tip of your nose is swollen two weeks after surgery is to be expected. This swelling will undergo a period of fluctuation for some time, and It is too early on in this process to make any prediction of eventual results. Suffice it to say, it will improve over time. Secondary rhinoplasties can take up to a year to heal. Try to be patient and rely upon...
Is it common or even possible to undergo a septal perforation repair and a cosmetic rhinoplast
Risky Challenge Requiring Expertise
Jan 20, 2018
Tricky challenge. While it is not common to repair a septal perforation at the the same time a cosmetic procedure is done, it is technically possible to do these procedures, together, depending upon the dimensions and location of the perforation. The smaller and most posteriorly located the perforation is, the easier it is to combine the procedures. In general, when combining a cosmetic procedure with closure of a perforation, the surgeon has to be extremely careful not to be too...
Would rhinoplasty fix my side profile?
The Perfect Candidate for Rhinoplasty
Jan 20, 2018
Yes, absolutely. The rhinoplasty operation is designed, specifically, to correct a profile such as yours. Your bulbous tip should be refined, also. I would recommend consulting with a board-certified plastic surgeon with extensive experience in rhinoplasty surgery. Nasal surgery outcomes are directly related to the amount of experience the surgeon has in performing the procedure. Prepare a list of questions you may have about the surgery, along with photos of noses you may like....
Is it time for a facelift?
Conservative Mid-Facelift
Jan 20, 2018
You're not alone. I hear this complaint often from new consultations. Obviously, ancillary forms of therapy like injectibles and Ultherapy have not worked for you. You are, however, an excellent candidate for a conservative short-scar mid-facelift. That procedure would most effectively address your concern.

What is the difference between a mini lift and a full lift?
Mini-facelift vs Full-facelift
Jan 20, 2018
A mini-lift addresses the mid-face, nasal labial fold and jowl areas. It is performed through a shorter incision and is a good operation for patients who don't need a lot of work on their neck. A full facelift utilizes a longer incision and can allow for the correction of more issues of the neck. Consult with a board-certified plastic surgeon for best results.

No exercise for two weeks
Jan 20, 2018
While you may desire to resume your exercise routine, you need to understand that you have had a hematoma and should do no activity which raises your blood pressure at all for two weeks. Get clearance from your surgeon before you resume such activity. The hardest thing about being a patient is being patient.

Breast Reduction with Breast Augmentation
Jan 20, 2018
You may be able to have both procedures done at the same time, If you want to stage it, you should allow 4-6 months for the first operation to heal because further revisions might be necessary along with the augmentation.

Nicotine and Suboxone
Jan 20, 2018
Absolutely stop the nicotine at least six weeks prior to procedure as it significantly increases the risk of complications of any procedure, and can adversely affect the outcome. Suboxone is an issue for the anesthesiologist (and must be discussed), and will affect both the anesthesia and pain management post-op. Be completely forthcoming with your physicians for best results.

Realistic Expectations
Jan 20, 2018
Your nose has some of the characteristics of the desired nose. That said, one should never pin their hopes on trying to have a nose that looks like another person's nose. This can only lead to ultimate dissatisfaction. Your nose can look closer to the desired outcome, but anothers nose can never be duplicated. As for the cost of such a procedure or payment plans, each individual doctor has their own fee schedule. In general, "you get what you pay for." Go with a...
