Good question, but that can only be answered if you were seen in person. That's the only way to assess how much breast tissue you have as well as the elasticity of the tissues to predict how you will heal and what size you may be. No one can promise or predict what you will be since the healing process can be unpredictable and different from patient to patient!

An "en bloc" excision is a cancer term - it means cutting out the cancer and taking normal tissue to make sure you get everything out. With capsulectomy, "en bloc" means you remove the capsule without any holes or tears. There are two issues. First, an en bloc removal is not technically possible in most patients because the capsule is so thin. First, it is your body and if you want your implants removed you should be able to find a qualified plastic surgeon willing to do that for you. It will be a big change again form a body image point of view, and you need to be counselled regarding what size you will probably wind up, whether you need a repeat lift to reposition your breasts, and also if you are a candidate for future fat grafting if you need /desire more volume. You can always get a second opinion!

From the photos alone, you have rather large breasts with ptosis (droop) which can esily lead to neck/back/shoulder pain. It appears your implants are riding high, which may mean capsular contracture (hardening of the implant capsule) and that can cause you pain also. For those reasons, I would consider removing your implants (and probably the capsules) as well as performing a breast lift to get your breasts back to a normal position and should help your pain issues.

If it is an explant only, especially a subglandular one, your recovery should be simple, easy and quick. That procedure can be done in the office under local anesthesia.

First, from your photo, it looks like you need a full (upside down T) mastopexy on both sides. And with breast asymmetry, I always use temporary sizers in the operating room before I put in permanent implants - this helps me decide exactly what size implant to put in to try to get a patient to the size they wanted. Also, make sure you fully understand all risks of putting in an implant, what you will need to do for follow up and most importantly that they are temporary devices only.
Can Seri Silk be successfully removed after implant removal?

If you feel a ridge/asymmetry between the breasts, it could be residual mesh material/Seri silk. Usually any mesh (even resorbable meshes and acellular dermal matrices like Alloderm) become incorporated and are part of the scar tissue/capsule that forms around an implant. If you remove the capsule completely, you will also be removing the mesh material, It is usually very obvious when you are performing a total capsulectomy.Brian Buinewicz MD

Yes, an implant removal only is straightforward to perform under local anesthesia. A total capsulectomy is more difficult to perform by far, and requires a general anesthesia. You really do not need to wait any specific time to have your capsules removed - without an implant in, the capsules may shrink and scar down (appearing like normal scar tissue). First, you are still very early in the healing process and your symptoms should resolve with time as you heal. Same with your breasts - it is still very early in the healing process. You should be starting to massage the breasts to improve the appearance, but check with your explanting plastic surgeon first.Also , there are medications like gabapentin that can be helpful with the postoperative burning pain - check with your surgeon also!
