Hello thank you for your question. Fat grafting is not advised, given the risks for fat necrosis. It is malpractice to do fat grafting because 30-50% of the fat will inevitably die and form clumps of dead fat that may hurt and cause unnecessary pain and worry with self monthly breast exams. Hello, thank you for your question. In my professional opinion, it is not advised to perform implant removal under local anesthesia. This is a relatively invasive procedure that should be completed with IV general sedation. I advise to finish nursing before you have removal of implant, capsule, and all inflamed tissue to ensure you can have the best optimal outcome for healing. Surgery can be done while nursing, but will pose an inconvenience for the mom.

Hello, thank you for your question! Unfortunately, there is no specific blood panel or test to look for complete removal of capsule and breast implant (saline & silicone). The ultimate way is to see what the post operative recovery is like, with freedom from breast implant illness symptoms. There is no imaging test to confirm this either. The goal of the surgery is to remove the entire implant, capsule, and all inflamed tissue, sincerely.

You are correct to worry about contamination within the chest wall if fluids travel elsewhere during the procedure. The entire implant, capsule, and all inflamed tissue must be removed as one system in EN BLOC fashion or 100% total capsulectomy. This will allow for optimal recovery. This is the gist of explant surgery. Not everyone can perform explant surgery.

Hello, thank you for your question. Each and every patient have different results aesthetically after surgery. Given your age, your skin would most likely recoil/fluff out after surgery—this can take up to a year to see full results. With this, I recommend to wait at least a year prior to having a breast lift after removal of breast implants. There are many risks with implant removal at the same time of a lift, including nipple necrosis and longer anesthesia times. Hope this helps!
