It sounds like you have two goals: one is larger breasts and the other is lower (and smaller) areola.

These goals are somewhat conflicting in their surgical treatment. Placing implants will inherently lift the nipple and areola higher and over time may stretch them wider.

Surgically lowering the nipple will require reducing the skin envelope of the lower breast, which often reduces the overall size of the breast.

Any open wound has bacteria in it and will produce drainage but may not be considered infected.

The best way to evaluate for an infection is to have an exam or to discuss your concerns with your treating physician who has seen your wound recently and knows the prior management.

Worsening pain, spreading redness, swelling, fever, foul odor and purulent drainage can all be signs of infection and should be addressed promptly.

Age alone is not a significant factor in results and healing time. The most significant factors will be your overall health status (whether you have conditions that effect healing, i.e. diabetes, collagen disorders, smoking) and the surgical approach. I would recommend consulting with a board certified plastic surgeon who could address these questions and can assess your skin elasticity and anticipated results.

Recurrent dysplastic nevi should be removed. The current medical understanding is that dysplasia represents and early pre-cancerous change. Your prior surgery appears to have been preformed by shave excision. Mole (nevus) cells tend to penetrate deeper into the skin than many shave excisions are able to remove, so probably some of the cells got left behind.

I would recommend returning to your surgeon or seeking a second opinion regarding re-excision of the mole.

In all likelihood removal under local anesthesia would be possible. An in-person exam and consultation would be needed to be sure you are a good candidate. From your pictures I cannot tell where your placement incision is located, but while you are breastfeeding, I would recommend removal via an inframammary incision. A breast lift will improve the shape and contour of your breasts, but it will inevitably leave scars. Your surgeon should be able to give you an idea of what type of scarring you can expect. There are a multitude of treatments to improve scar appearance and prevent "ugly" scars in most cases. I recommend scheduling a consultation with a reputable breast surgeon who is certified by the American Board of Plastic Surgery.

This early post operatively there is a lot of swelling and bruising. If the tissues are warm and swollen, the skin will very often heal very well. Even with a small amount of skin loss of the areola, the final outcome is typically excellent. I typically recommend Aquaphor for any dark or bruised appearing regions of the nipple or areola. Talk to your surgeon about your concerns, but be reassured that things appear to be doing well at this point.

Your folds don't strike me as overly prominent, but if it is bothering you it is worth looking into solutions. Unfortunately there are no creams or lasers that will improve the depth of your folds. For a shave excision this can be normal healing. However, with redness around the edges, it may be infected. You should follow up with your physician as soon as possible so he or she can examine the wound and determine if antibiotics would be appropriate.

Most women have fully developed breasts by age 16 or 17. Following that time the breasts get larger related to changes in overall body weight and pregnancy. The symptoms you describe make you a good candidate for a breast reduction. Depending on the type of breast reduction there may be risks to future breast feeding.
