There is always swelling / edema after having abdominoplasty surgery. Much of the vascular supply and lymphatics are interrupted once the surgery is complete and can take anywhere above six months to "regrow" and sometimes closer to a year. The abdominal binder helps to squeeze out some of this fluid and although it is recommended to wear for a month to six weeks, I find many patients feel more comfortable wearing it longer.

Every reconstructive case is handled differently, depending on the breast size, expected residual tissue, and status of adjacent tissue that may be used to transfer to recreate your new breasts (abdomen and back).
Certainly if there is an abundance of skin, then an expander may not be necessary and immediate one stage can be performed. Otherwise the use of expanders, TRAM flap, or latissimus flaps are great options. Again, each case is individual.

Thanks for submitting your photos and questions. I am sure you are helping others who have the same questions.
I always tell patients that they shouldn't be too concerned over the first few weeks. Immediately after surgery, the tissue will sell and the contour and incisions will be altered. I tell patients they will look "good" by a month but so much better by three months.
You incisions appear typical for a week post op and as the swelling goes down and the slight bunching...

I do feel that you are a good candidate for a "belt Lipectomy" which will be a life changing procedure. The fact that you are still losing weight and emotionally are set to lose more, I would hold off. The reason being that when you were heavier you lose elasticity over time and if you have the surgery and then lose another 40 pounds that may leave you with additional excess skin.
