Healing after basil cell removal. Flap, graft or natural healing? Natural healing is preferred.
A flap is the best solution
Jan 20, 2016
Dear sralston,Hopefully the basal cell cancer was removed using mohs surgery which examines the entire margin, and has the highest cure rate. A flap--moving adjacent skin to fill the defect-- would be the most appropriate method of reconstruction. If it heals naturally, there is a high risk of pulling on your mom's lower eyelid with the wound contraction. A skin graft would create a poor color and texture match. While there will be a scar from the reconstruction, your surgeon will design it...
Can I hide a mini lift from my roommate?
Hiding a mini-lift
Jan 17, 2016
Dear Tarije,If you have long hair, you can definitely hide the mini-lift scar. For bruising, the appropriate make up or foundation will camouflage it. Swelling is more challenging. Diligent icing will decrease swelling, but it will probably still be noticeable. Often swelling from a mini facelift can be similar to that of having several wisdom teeth removed.
Feeling a twitch under my healing scar after having mohs surgery on my face
Twitching is normal for early scars
Jan 17, 2016
Dear Jen,Twitching also known as parathesias is normal for scars whether from mohs surgery or other types of surgeries. It is a symptom of the skin nerves growing back and connecting. This may occur for the first 6 months or more. Massaging the scar may help.
What is the least invasive treatment for a small BCC?
Small BCC near the eye
Dec 12, 2015
Mohs surgery is the gold standard since it examines the entire margin, has the highest cure rate for nonmelanoma skin cancer and may remove less skin. Many mohs surgeons are very skilled at reconstruction of these types of defects, and there are many methods to optimize scars from silicone gel to dermabrasion to lasers. There are other therapies that are effective such as radiation in particular electronic brachytherapy but it currently doesn't have the long term evidence of its...
When will the swelling resolve?? When should I start exercising again??
Swelling
Dec 12, 2015
Swelling is common with mohs surgery on the forehead and the subsequent repair. In some patients, it can be quite significant in the forehead and upper face because of gravity and what we is known as potential space. Everyone reacts a bit differently, but it may take several weeks to fully resolve. The first 3-4 days after mohs surgery, there can be a high risk of bleeding so exercise may not be prudent. Your body will let you know when it is ready to exercise, but generally after a...
How many types of skin grafts are there?
Skin Grafts
Dec 12, 2015
Skin grafts are generally divided between full thickness skin grafts and split thickness skin grafts. Full thickness skin grafts are usually used for facial skin cancer defects created by mohs surgery. Split thickness skin grafts are generally used for large defects on the trunk or extremities. Donor sites for full thickness skin grafts are commonly taken from behind or in front of the ear. Split thickness skin grafts can be taken form the thigh or buttocks.
Am I a candidate for a mini facelift? If not, what are my other options?
Is a mini lift approrpiate
Sep 20, 2015
Thank you for sharing your photos. Though you would need a full history and physical exam to accurately assess your needs, it does appear that you would benefit from a mini lift. Because when I do a mini lift, it only improves jowls, the lower and midface sagging, I almost always include neck liposuction which will help with a double chin. I perform my minilifts using tumescent anesthesia and oral sedation. The procedure takes about 90 minutes. With that said there are non surgical...
BCC removed. The area is closed but still red. Can I swim laps in a chlorinated pool which is my
Swiimming after Surgery
Sep 20, 2015
Though I can't specifically answer your question since you are not my patient, if a surgery site is still red and irritated, chlorine will only inflame it more. It is best to have the physician who performed the surgery examine your nose and make that decision.
I feel the Moh's "surgeon" went way too deep/wide on his first stage. Can you look at my before/
Mohs Surgery For Basal Cell Carcinoma
Sep 20, 2015
It is difficult to specifically comment on your situation without seeing you as a patient. However, the American Academy of Dermatology, the American College of Mohs Surgery and the American Society of Dermatologic Surgery have a developed Appropriate Use Criteria for Mohs Surgery--guidelines for when to use mohs surgery. Generally, for a BCC, a mohs surgeon will take a 2 mm margin around what he/she believes to be the clinical extent of the tumor. When a defect is made in the skin because...
Can Flap Surgery ( Moh's Surgery) work for acne scars?
Surgery for Acne Scars
Sep 20, 2015
Mohs Surgery is used for skin cancers, though it has been reportedly used for some other conditions but not acne scars. The most common surgical procedures for acne scars are punch grafts--removing the depressed (usually an ice pick scar) with a small tool that is also used for biopsies and then the defect is sewed together with stiches.
Could you tell me if a heating pad would relieve the scarred incision following Mohs surgery aft
Many way to improve surgical scars
Aug 26, 2015
Scars can take one year to mature. Scars can be optimized by massaging in OTC silicone gel twice per day for several months. If the scar is raised, a steroid injection may flatten it. If it is red, then a laser such as a pulsed dye laser may decrease the redness--though it will require several treatments. More recently, it has been shown that fractional laser resurfacing can greatly improve scars.
I am 55 and considering a facelift, but am a bit scared. Is there a mini-lift I can do?
Mini vs full lower facelift
Aug 23, 2015
A mini lift generally involves a short incision along the front of the ear. It can be performed with local anesthesia and combined with other procedures such as blepharoplasty (eyelid lifts) or laser resurfacing. It lifts the mid and lower face but not the neck--though I always include neck liposuction with this procedure. So, it will help if you have jowls, but not if you have excess neck skin and/or neck bands, If you have both jowls and loose neck skin, then a full lower facelift...
