When I exchange implants and decrease in size this much, I always talk with the patient about a lift as well. The skin envelope needs to be made smaller, or it will be redundant and the implant won’t fill it out as well as the previous one did. Of course this would require scars and some patients are unwilling to proceed with the lift for that reason.

During your consultation your surgeon should be discussing your goals with you and giving some options for how to achieve them. I think your preferences should always be considered, but a Board Certified Plastic Surgeon will also balance the safety and efficacy of the placement/procedure with your preferences. It sounds like you may be a good candidate for something like a breast lift or mastopexy to achieve your goals, perhaps in combination with fat grafting.

I would make your operating surgeon aware of this change as soon as possible. Periprosthetic infections are difficult to treat, and in my practice I would take you back to surgery urgently for this drainage, which may represent infection.

In my practice I prefer for patients to be nicotine-free 6 weeks prior to surgery and 6 weeks after. I usually get urine cotinine testing at pre-op visit to ensure there are no further nicotine metabolites in the system! Vapes are unregulated and thus even vapes advertised as nicotine-free can be problematic in that some of them do include nicotine (evidenced on positive cotinine testing some of my patients). I would recommend avoiding all vapes 6 weeks before and 6 weeks after for best healing.
