It does appear that you have had some recurrence of prominence of your ears. If it has been 3-6 months from surgery, which I think it has, then I would revisit this with your surgeon. I would typically consider revision about 6 months after the initial surgery if there is recurrence.

Thank you for your question. This does not appear to be a Stahl's deformity. It is difficult to tell from this single view if the problem could be fixed surgically. It appears that the curve of the helical rim or edge of the ear is a bit different causing the shape of the ear to not be smooth. An examination in person would really help you to know if this could be addressed with an operation.

Thank you for your question. Given that your surgery was only 2 weeks ago, it is too early to know what the final result will be. Typically, you need to wait until at least 6 weeks out from surgery to see an early result. It can take several months for all the swelling to go down. Thank you for your question. It is very normal to have swelling a month after TOP surgery. The final result is usually not seen for at least 6 months after surgery. I typically tell my patients not to do any heavy lifting or strenuous activity until at least 6 weeks after surgery. In general I wait for about 6 months after the peri approach to allow the nipple and areola to shrink after the breast tissue is removed. If the nipple is still large, then I go ahead and reduce the nipple size. Based on my experience, I think you are a good candidate for a periareolar approach. You have good skin tone and the size of your breasts lends itself to this approach. You will have minimal scarring and your nipple and areola will get smaller when the breast tissue is removed. What you see is a combination of less developed breasts and a constricted breast deformity. I wouldn't call it tuberous per say. This isn't ptosis either. Ptosis is when the nipple is sitting low on the chest relative to the breast fold, this is not the case with you. It is absolutely possible to create a subtle fold. It makes sense to see your surgeon to have them look at and examine your ears to determine how stiff the cartilage is and what would work best. Often this type of surgery requires over correction because the ear wants to go back to where it was before surgery. Therefore the ear may look like it is more “pinned back” initially and will relax with time.

A small amount of asymmetry is not unusual after otoplasty. Looking from the front, the ears should look very similar. If one is a lot more prominent than the other, then revision might be needed. Usually I advise patients to wait at least 6-12 months after surgery for all the swelling to settle and the ears to relax a bit before considering revision. By 3-6 months the position is close to where it will be long term.

It is not uncommon for one ear to project more than the other. Often this is caused by decreased folds in the cartilage of the ear on that side. This can be corrected with a unilateral otoplasty where one ear is repositioned closer to the scalp via a well hidden scar on the back of the ear. Sometimes both ears can be repositioned more posteriorly to give a more symmetric look. Your surgeon will determine this after they examine your ears and look at the cartilage folds.
