Unfortunately, your frame will determine what implant you can accommodate. That being said, the implant sizes you have stated seem a bit small. The breast width will determine what size your limited too and there are more narrow implants with more projection and cc’s for that matter. In general I determine the size before consultation and rarely need to change sizes at the time of surgery.

Swelling is unusual 10 years after augmentation. I would follow up with your plastic surgeon and do not delay. It could be inflammation of the veins that drain the breast (thrombophlebitis) however there is usually pain associated with it. You want to be sure that there is nothing ominous with breast since it’s only happening to one side.

It’s not likely that your surgeon did anything wrong. He/she should have explained to you that this could be a possibility if your areolas were quite large. This is more common with Breast reduction and is fixed with a follow up scar revision. Do not hesitate to get them revised.

Judging by your picture, I would not use a moderate profile implant especially since you will need a larger implant. Despite having broader shoulders, your breasts are a bit constricted meaning you won’t have enough tissue to expand over a such an implant. You will not be able to lift your breasts with fat transfer alone. Based on your picture, you will require incisions to rearrange the Breast tissue. Generally, a vertical (or lollipop) oriented scar will be present in order to provide the result you desire. Fat grafting can be used to give you more upper pole fullness.

If you see a significant difference in size, then the implant has likely ruptured. If you can still feel the implant, but the position is different, then you may have changed the pocket that the surgeon created for your implant. It wouldn’t necessarily be a pulled muscle that would cause displacement of an implant. Please follow up with your surgeon as soon as possible to get checked out. Good luck.

I generally tell my patients that if your breast size changed significantly during pregnancy, then you should hold off as sagging could affect your ultimate result. With you being athletic you would likely need a sub muscular implant if you do not have much body fat. You can achieve a nice result with an augmentation mastopexy. The areolas can be reduced and an implant will provide the roundness and fullness you are looking for. Unfortunately, you will need a vertical incision ( lollipop) in order to obtain a proper lift.
