Funny how urban myths can assume credibility. As my colleagues below and above have / will state, the only known safe materials to make a human breast bigger are gel or saline implants, or your own transplanted fat.

Don’t get too stuck on details like drains. Considering a moderately big operation like Abdominoplasty and liposuction, the burden of drains is a small part of the total recovery after surgery. Think more about wound care, nutrition, pain control, clot prevention, and activity. Like planning for a baby, planning for cosmetic surgery should be reasonably predictable. One or two drains is commonly done, lasting about one or two weeks.

Unless the volume of remaining filler is thick and palpable, I don’t think recent filler therapy will influence the performance of lower blepharoplasty by your plastic surgeon. Most of the tissue manipulation is mid-lid, between the lash line and the bony lower orbital rim. The tear trough is at the lowest part of the blepharoplasty field.

Combining two moderate operations is a matter of scale, proportion, and complexity. We always weigh efficiency of the time savings of combining operations versus the increased risk and stress and possibly tougher recovery.
If it patient’s ambitions for augmentation are modest and she is very suitable for an abdominoplasty, i.e., good muscle tone and largely lax skin without a great deal of fat, then the operations being combined is generally a pretty good fit…

I think that augmentation at too young an age can be unsatisfying over the long term. First, the chest wall and breast tissues can still be growing and maturing. The expectation of a large-ish implant under immature tissues can actually cause atrophy and thinning of the overlying tissues, leading to faster fatigue of the tissues and loss of elasticity. Most colleague plastic surgeons will waffle on this seemingly simple question, myself included. I specifically state that the purpose of breast reduction is reduction in symptoms, such as chronic back neck and shoulder pain & rashes underneath the breasts, not a specific bra size.

The usual result from breast reduction is an upward lifting, compaction of looseness, and reduction in overall volume. There are quite a few ways to do this but most involve removing excess skin and internal breast tissue, and repositioning the nipple to be directed forward rather than downward. The best method for you is difficult to specify sight unseen. Good luck in your journey to better comfort and mobility.

After breast augmentation, the breasts will become moderately swollen and quite a bit firmer than normal, both from the presence of the addition volume of the implant, and also from the general puffiness that accompanies handling / traumatizing the chest wall tissues. The general direction the breast goes is less firmness, with relatively small decreases in absolute size as healing progresses.

For primary breast augmentation, I ask the anesthesiologist to provide general anesthesia with airway control, usually a LMA tube. This permits me to focus on optimizing the operation, content that the patient is safely ventilated.

One condition to treat is the torn earlobe or the multiply pierced earlobe. Depending upon the degree of tear or multiplicity of pierce holes, the first goal is to convert the earlobes to a simpler, less pierced or unpierced state. The first time I treat a patient for crow's feet in the area of the lateral lower eyelid, I usually start with 5-6 units per side, spread over two or three injection sites. The dosage can be raised for more dramatic and sustained effect, but it is important not to weaken the muscle that crinkles the eye so much that the lower lid sags, which can be uncomfortable or unattractive.

I generally see patients having a return of overactive muscles of the brow, forehead, or crow’s feet, at around 4- 6 months after treatment. Over time and multiple repeat treatments, there is often a longer period of effectiveness, but the same dosing of botox is needed, just further apart in time.
