The jowl is actually a fat pad and will enlarge with weight gain; it also becomes more prominent with aging partly because facial volume in adjacent areas of the cheek can diminish relative to the jowl. For lower lid blepharoplasty I first do a block of the infra-orbital nerve (this usually numbs most of the lower lid) using 0.5% bupivacaine (Marcaine) with epinephrine. This is followed by direct injection of 2% lidocaine (Xylocaine) with epinephrine into the lower eyelid area. The epinephrine constricts small blood vessels, which prolongs the effect of the local anesthetic and reduces intra-operative bleeding and bruising. Thanks for your question. Your tattoo includes some "exotic" colors that include less-commonly used inks. It may be difficult to achieve 100% clearance of this tattoo, mainly because yellow tends to be pretty unresponsive to laser treatment.Of the colors we see, black is the most responsive and usually clears completely with both 1064 nm and 755 nm laser wavelengths. Great question! As I discuss in my book “Understanding Cosmetic Laser Surgery” multi-colored tattoos like this one may require the use of two or more laser wavelengths for complete clearance. Because the FDA does not regulate tattoo inks, it can be difficult to predict exactly how well tattoo inks will respond to treatment with a given laser wavelength.

Thanks for your question and photos. You have fairly prominent excess orbital fat, which causes bulging in the lower eyelid area. The bulging also appears to be a bit uneven, with more on your left side than on the right. The best solution is to remove the excess orbital fat; this is what is done by a lower lid blepharoplasty. A filler injection into the "tear troughs" would also help but would only address the more medial area between the eyelid and nose (see accompanying video).

Alyssa:Picosecond laser treatments will provide the quickest tattoo removal. The blue color may respond best to the PiQo4 laser (1064 nm). If there is any green (or blue-green) the PicoSure laser (755 nm) would likely work better. Yes it is possible to treat just sections of a tattoo without treating the entire tattoo. Lasers are quite precise and when focused to a small spot size can treat small parts of the tattoo, such as the lines you described.
