Hi emily58,Based on your photos and background information I think you would be best suited with an abdominoplasty with liposuction. As you mentioned I think you'd end up with excess skin if you did lipo alone. Hi jo_toronto,Unfortunately, an umbilical hernia repair will not address the loose skin and stretchmarks. This problem is seen commonly after having several children. An abdominoplasty will address the problems you have....meaning we can fix the umbilical hernia, remove a lot of the stretch marks in the lower abdomen, repair the diastasis (or separated 6 pack muscles) and remove the excess lower abdominal skin. Without seeing you and examining you it is hard to really weigh in on it....but if you have a bulge in the upper abdomen and your surgeon says you likely tore some of the sutures then you likely do have a recurrence of your DR. The good news is since you have a FDL vertical scar it can be opened and the muscles addressed directly instead of going through your horizontal incision and having to dissect all the way up to the upper abdomen. Interesting question. I'll start by saying after having had a midline abdominal incision for colon cancer surgery you likely have a weakness in the form of a ventral hernia. That would be the most common problem. Unfortunately, after midline incisions is is very common to have a weakness/hernia that reveals itself down the road. I agree with your assessment. Again, without seeing you doing a good history and physical exam it is hard to give you a lot of useful information....but the area that you are describing is more consistent with a hernia (inguinal or femoral). This "odd pressure" you feel may be a weakness in the abdominal wall and not a true hernia but over time could eventually become a hernia. Or it could be a pulled muscle, or ovarian pain, or.......Having said all that I recommend...

I think you're looking for a radiologist. Having said that a CT scan may not be helpful in the determination of a diastasis or separation of your abdominal wall muscles. It is a great study and provides a wealth of information but it is performed while lying down so there is no pressure on the abdominal wall....so it could miss a subtle widening of the rectus (abdominal wall) muscles.
